Affordable Care Act: Our certified ACA counselors will help you with your insurance application on Healthcare.gov.

Medication Assistance Program:  Meet with our medication assistant program coordinator to apply for medication assistance to obtain your medication.

Patient Portal: A Patient Portal is a secure online website that gives you convenient 24-hour access to your personal health information and medical records.

Referral Coordination: When your provider refers you to see a specialist or to get a specialty test our Referral Specialist will help coordinate your appointment with you up to but not limited to the following:

 

• Medicaid

• Medicare

• Private Insurances, including Blue Cross/Blue Shield, Coventry(Aetna),

• Self Payment, based on a sliding fee scale*

 

We offer a sliding fee discount program. Based on your income and household size we are able to apply a discount to your visit. On the first visit to the clinic, please bring proof of your income and family size to help us determine your discount.
