Job Openings

Under the direct supervision of the Director of Clinical Services (DOCS), the County Nurse Manager (CNM) is responsible for the day-to-day effective and efficient operation of the assigned practice’s clinical operations. The CNM ensures that staffing needs are met, supplies are available and accessible to the care team and that quality patient care and customer satisfaction remain central to the activities of clinical employees at the practice site. Core job duties are operational, clinical, clerical and supervisory. Under the supervision of the DOCS, the CNM assists to facilitate patient care in the clinical environment. S/He is an essential part of the care team from the onset of the visit until its completion. The CNM will be involved in both the clinical and administrative areas. The CNM is responsible for ensuring all MAs are capturing and documenting clinical and demographic information accurately in the Electronic Medical Record (EMR). S/He performs as training and vaccination training as needed to staff. The CNM also coordinates and assists with purchasing and management of medical supplies as directed by DOCS. S/He communicates directly with the DOCS for direction and information for clinical flow and procedures.

PT Pediatrician

Provides professional medical services to complement the delivery of primary health care services at Cabarrus Rowan Community Health Centers.  The role of the Pediatrician is to assess, maintain and improve the health status of children in the communities served.  The Pediatrician provides indirect supervision of medical support staff.

Interpreter

Act as conduit between patients and medical/behavioral professionals and/or office staff by listening to the CRCHC staff and orally translating information into a patient's native language in layman's terms. The medical interpreter then translates the patient's response back to the professional, using proper medical terminology. Interpreters may translate such information as the reason for a medical visit, past medical history and family medical history, as well as explaining medical and surgical procedures, giving medical care instructions, providing drug information and scheduling follow-up appointments. In providing translation services for the clerical staff, the interpreter may translate all aspect of required for appropriately capturing demographics, billing and pre-clinical coding information. This role collaborates with health care team to ensure clinicians obtain accurate information and patient/family are fully informed. Interpreters are also responsible for anticipating and helping staff avoid culturally inappropriate interactions with clients served while always protecting patient privacy in the execution of his/he job duties.

Clinical Supervisor

Under the supervision of the Director of Clinical Services, the Clinical Supervisor will assist with the triage of patients on-site and via telephone utilizing approved protocols or standing orders.  The Clinical Supervisor helps to facilitate patient care and clinic flow in the clinical environment.  The Clinical Supervisor may be involved in both the clinical and administrative areas.   The Clinical Supervisor will document provider orders, clinical actions, and interventions in the electronic health record.  S/He communicates directly with the Director of Clinical Services for direction and information for clinical flow and procedures.

The Care Coordinator will assist patients in navigating the care among primary care, behavioral health, specialists, and other members of the integrated care team. The staff member will assist with appointment scheduling and follow-up, disease management, community resources, and patient advocacy. With input from the clinical team and data from the EMR, he/she will identify high risk or utilization patients for additional support and care plan development. The Care Coordination will also serve as additional clinical or clerical support for the facilitation of care transitions. Other duties may be assigned to suit the demands of patients, according to the respective location of the clinical site.

Float CMA

 Under the supervision of the Director of Clinical Services (DOCS) and the assigned provider, the Certified/Registered Medical Assistant (CMA/RMA) helps to facilitate patient care in the clinic environment. S/He is an essential part of the care team from the onset of the visit until its completion. The CMA/RMA may be involved in both the clinical and administrative areas. S/He is one of the primary guides for the patient through the appointment process. Tasks assigned to this role are both clinical and clerical. CMA/RMAs are responsible for capturing and documenting clinical and demographic information and accurately documenting this material in the electronic health record. CMA/RMAs also assist providers with procedures and communicate with patients in various forms and multiple settings.

Patient Representative 

Under the direct supervision of the Site Coordinator, this position is responsible for timely and accurate recording of patient demographics, insurance information, patient charges and payments. The Patient Representative facilitates patient entry into Cabarrus Rowan Community Health Centers (CRCHC), including greeting client, answering phone calls, and scheduling appointments.

Physician

Provides professional medical services to complement the delivery of primary health care services at Cabarrus Rowan Community Health Centers through a PCMH model of care.  The role of the physician is to assess, maintain and improve the health status of individuals in the communities served.  The physician provides clinical leadership and supervision of mid-level providers and indirect supervision of medical support staff.

Licensed Clinical Social Worker

The Licensed Clinical Social Worker Associate (LCSWA) Clinician provides direct clinical social work services to patients and families to improve and promote overall health, with a focus on addressing the behaviors that impede optimum health.  This role coordinates services with the interdisciplinary team to provide consultation regarding mental health and/or substance use/abuse counseling while facilitating the development of a comprehensive plan of care to address the whole person.  The LCSWA adheres to all internal policies and practices, in accordance with widely accepted professional practice standards, and maintains requirements for professional practice and licensure. 

 The Call Center Representative (also “CCR”) is responsible for providing high quality service to all individuals that contact our office via telephone or the internet. The CCR ensures that all patients receive access to care and information via telecommunications and online methods, as appropriate. The CCR assists the supervisor and/or manager with a variety of general office activities and may be asked to assist in various front desk operations among all sites when necessary. The CCR will assist with patient requests, all incoming communications to the Center, scheduling patient appointment requests, call routing, outbound contact initiatives, and screening incoming communications all while ensuring the delivery of excellence in customer service.

Site Coordinator

The Site Coordinator is responsible for organizing and coordinating office operations and procedures in order to ensure organizational effectiveness and efficiency. Supports mission and goal achievement, organizational quality and patient-centered care. Incorporate core organizational values of quality, respect, integrity, partnership and compassion in all activities and decisions. Plan, develop, organize, evaluate, and direct front office clinic site activities under the direction of the Operations Manager.

Clinical Supervisor

Dental Assistant prepare for oral examination and assist other dental workers, such as dentists, dental therapists and dental hygienist, in providing treatment to the teeth, mouth and gums. Dental Assistants also carry out chair-side and receptionist duties.

S/He is an essential part of the care team from the onset of the visit until its completion. The MA may be involved in both the clinical and administrative areas. S/He is one of the primary guides for the patient through the appointment process. Tasks assigned to this role are both clinical and clerical. MAs are responsible for capturing and documenting clinical and demographic information and accurately documenting this material in the electronic health record.  MAs also assist providers with procedures and communicate with patients in various forms and multiple settings.
